Preventable chronic disease (PCD) information and self-management support that health services provide to Aboriginal people with PCD in remote communities of Northern Territory has not been sufficient to date for enabling those people to adequately manage and/or reduce the incidence of PCD. - Preventable Chronic Diseases [PCD] such as cardiovascular disease, diabetes, renal and respiratory disease are a major cause of death and disability worldwide (Michie et al. 2008). PCD is endemic in the Northern Territory.
- There are many people with high Hba1c's documented on chronic disease databases.
- DHF want to trial models of group education for individuals and families with diabetes and vascular disease that support self-management.
- Group education and disease management models for Aboriginal people have not been explored or evaluated rigorously in the Northern Territiory.
